The Service Support Specialist is responsible for coordinating and supporting Direct Care Workers, managing referrals, and fostering community relationships to ensure quality home and community-based services. This part-time role involves intake assessments, service planning, scheduling, and monitoring compliance with care standards. The position requires bilingual skills, supervisory experience, and a commitment to empowering individuals to live independently.

As a Service Support Specialist, you’ll be responsible for receiving, accepting and documenting all new referrals and developing and maintaining relationships within the community. You will also coordinate the Direct Care Workers (DCW) from inception to assignment and beyond, ensuring services are provided in a manner that is consistent with our mission.
